Learn from a completed project while the details are fresh
The end of a project is a useful moment to collect lessons before everyone moves on. A short review can preserve practical improvements without turning into a long meeting about who deserves credit or blame.
Compare the plan with the experience
Look at what you intended to deliver, what was delivered and which assumptions changed. Invite people who did the work to describe one thing that helped and one thing that created avoidable friction. Use specific examples, such as unclear approval ownership or a useful shared checklist. Keep personal criticism out of the record. If the review concerns a sensitive conflict, arrange an appropriate separate conversation instead of making the group meeting carry it.
Turn a lesson into an owner and an action
A list of observations is easy to file away and forget. Choose one or two improvements that can be tested on the next project. Name who will update the template, change the briefing process or clarify the handover. Preserve successful practices too. Send a concise record to the participants and check that it represents the discussion fairly. The review is finished when its useful lessons have somewhere to go.

- Assign an owner and a place to record the revised process.
